When you receive an approval request via email just click on the hyperlink to open the Review and Approval Web Page:
Select the Note tool and click on the place in the file where you want to add a Comment; write your comments into the box that pops-up. Or, use other tools to highlight areas of the file.
View the comments made by other reviewers.
Of course, you can reply to comments made by other reviewers.
